Case Studies

A 42 year old Iraqi male started having pain around left knee joint two years ago followed by appearance of a swelling. He was diagnosed with liposarcoma of left thigh which was excised in his hometown. Five months after the procedure he re-developed a bigger swelling in left thigh and underwent another resection followed by a course of radiotherapy. After one year he started having abdominal pain and symptoms suggestive of obstruction. He underwent exploratory laparotomy with excision mass. Despite these measures he persisted to have symptoms and hence was referred to our team. He was re-evaluated and found to be having recurrences in abdomen and left groin. The abdominal recurrence was an extensive disease with multiple tumors. The patient underwent exploratory laparotomy with complete clearance of the macroscopic disease. This clearance involved a distal pancreatectomy and splenectomy, clearance of the subhepatic portal region and a transverse colectomy with a primary anastomosis without a covering ileostomy
A 55 year old male patient presented with insidious onset jaundice of two months duration associated with pruritis and weight loss. His liver function test showed direct hyperbilirubinemia of 7.2 mg/dl and raised alkaline phosphatase of 637 IU. He underwent an ERCP evaluation which showed a periampullary growth measuring 2 cm in size. A biopsy from the growth was positive for adenocarcinoma. He was staged using a contrast-enhanced CT abdomen with arterial phase. The CT scan showed classical double duct sign with a periampullary tumor. Once his operability was confirmed he underwent an uneventful Whipple’s Pancreatico-duodenectomy procedure.
A 69 year old female presented with dysphagia of six months duration. She was a known case of carcinoma breast. She was first detected with carcinoma left breast fifteen years ago when she underwent mastectomy followed by chemo-radiation. She developed right breast carcinoma one year ago for she underwent only mastectomy. An upper GI endoscopy showed a narrowing in her mid-esophagous and biopsy proved it to be squamous cell carcinoma. Her PET CT scan was negative for any metastasis. She underwent a laparoscopic transhiatal total esophagectomy with gastric pull-up. She had an uneventful post-operative recovery and was discharged on the fifth post operative day.
